Claim Submissions

The correct address for the submission of paper claims and correspondence can be found on the back of the member’s identification card.   

Paper claims should be submitted to Total Claims Solution, Inc (TCS) on the standard CMS1500 or UB92 claim forms.  For general forms, guidelines and instructions please visit The Center for Medicare and Medicaid Service’s website at: www.cms.hhs.gov/forms.
